About this Role

The RN Case Manager Discharge Planner will play a critical role in ensuring a smooth transition of care for patients. This individual will evaluate the timeliness and appropriateness of patient care, coordinate discharge planning, and identify patients with complex needs.

Responsibilities
  • Evaluate patient care and develop discharge plans in collaboration with physicians and other healthcare providers
  • Identify patients with complex discharge planning needs and recommend post-hospital care options
  • Work with patients, families, and healthcare teams to ensure a seamless transition of care
Requirements
  • Licensed to practice as a Registered Nurse in the State of Maryland
  • Graduate of an accredited Nursing program
  • Bachelor's Degree preferred or equivalent combination of education and experience
  • Minimum of 2 years clinical experience in acute care setting, with experience in care coordination and discharge planning
