This PRN Weekend RN Case Manager position is responsible for coordinating patient care and services in a hospital setting. The ideal candidate will have a strong background in nursing and experience in case management.
Responsibilities
Coordinate patient care and services in a hospital setting
Develop and implement individualized care plans for patients
Collaborate with healthcare teams to ensure seamless patient care
Communicate effectively with patients, families, and healthcare teams
Requirements
Licensed to practice as a Registered Nurse in the State of Maryland
Graduate of an accredited Nursing program
Bachelor's Degree or an equivalent combination of education and experience
Minimum of 2-5 years clinical experience in acute care setting
Current CPR Certification required
