Job Summary

We are seeking a highly skilled and compassionate Outpatient Therapist to join our team at CommuniCare. As a key member of our treatment team, you will provide evidence-based services to individuals in their residences or community settings.

Responsibilities
  • Provide high-quality, patient-centered care while maintaining fidelity to established treatment plans.
  • Collaborate with clients, families, and other healthcare professionals to ensure seamless care coordination.
  • Develop and implement individualized treatment plans that address the unique needs of each client.
  • Conduct regular assessments and evaluations to monitor client progress and adjust treatment plans as needed.
  • Participate in training and professional development opportunities to enhance clinical skills and knowledge.
  • Meet all relevant standards and policies regarding patient care and documentation.
Requirements
  • Master's degree in a related field (counseling, psychology, social work, etc.) and licensure or certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver's license.
