About the Role

We are seeking a skilled and compassionate Certified Patient Care Specialist to join our team at ResCare Community Living.

Job Summary

The Certified Patient Care Specialist will be responsible for providing high-quality patient care, ensuring the dignity and independence of clients, and maintaining accurate records of patient progress.

Key Responsibilities
  • Provide 1:1 care for clients, following individualized plans of care and maintaining client dignity and independence.
  • Monitor clients' vital statistics and report any abnormalities to case managers and physicians.
  • Administer medications, perform feedings, oxygen therapy, and ostomy care as needed, and document results.
  • Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
  • Ensure infection control policies are maintained, report possible communicable diseases or infections, and implement infection control procedures.
  • Accurately document nursing actions and communicate with family members, case managers, and physicians regarding patient progress and outcomes.
Requirements

To be successful in this role, you must have:

  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current licensure as an LPN or LVN in good standing from the state(s) of practice.
  • CPR certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
  • Familiarity with symptoms and signs of improvement for various medical conditions.
