About the Role

We are seeking a highly skilled Social Worker to join our team at Harborview Medical Center. As a Social Worker - Inpatient Care Management, you will play a critical role in providing direct social work services to patients in our inpatient medical and surgical units.

Key Responsibilities
  • Provide specialty consultation to healthcare staff and teams regarding psychosocial aspects of patient care
  • Offer psychosocial support to patients and families
  • Collaborate with the multidisciplinary team to develop and implement safe and timely discharge plans
  • Provide care coordination for patients with complex social and medical histories
  • Offer case management and counseling services in specialized programs
  • Make timely referrals to hospital and community agencies as required
  • Perform related duties as assigned
Requirements
  • A Master of Social Work degree from a program accredited by the Council on Social Work Education
  • One year of full-time social work experience in a healthcare setting or equivalent

Within 90 days of hire, you must hold one of the following credentials issued by the state of Washington:

  • Agency Affiliated Counselor
  • Licensed Social Work Associate - Advanced
  • Licensed Social Work Associate - Independent Clinical
  • Licensed Advanced Social Worker
  • Licensed Independent Clinical Social Worker
