About the RoleAs a Case Management RN PRN, you will play a vital role in assessing, planning, implementing, and evaluating the care of patients in our Advanced Wound Care Center. You will work closely with physicians to develop and implement wound treatment plans, participate in performance improvement review activities, and promote a supportive environment that encourages teamwork.
Responsibilities- Assess patients' physical and emotional needs, develop individualized care plans, and implement evidence-based interventions.
- Collaborate with interdisciplinary teams to coordinate patient care, ensure continuity of care, and promote optimal outcomes.
- Participate in performance improvement initiatives, identify areas for improvement, and implement changes to enhance patient care and satisfaction.
- Provide education and support to patients and families, promoting self-care and empowerment.
- Stay up-to-date with the latest research, best practices, and regulatory requirements, applying this knowledge to improve patient care and outcomes.
- Graduate of an accredited School of Professional Nursing.
- Current RN licensure with the Tennessee State Board of Nursing.
- Minimum of 2 years med-surg experience, with wound care experience preferred.
