Job Summary

Ascension Saint Thomas is seeking a skilled RN Case Manager to coordinate the overall interdisciplinary plan of care for patients. As a key member of the care team, you will create and implement plans for care across the continuum, integrating patient and family preferences and values. Your expertise will be essential in monitoring patient care through assessments, evaluations, and patient records, advocating for resources and removing barriers to care. You will also maintain ongoing dialogue with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans. This is a full-time, day shift position at St. Thomas Rutherford Hospital in Murfreesboro, TN.

Responsibilities

* Coordinate interdisciplinary plan of care for patients from admission to discharge
* Create plan for care across the continuum, integrating patient and family preferences and values
* Monitor patient care through assessments, evaluations, and patient records
* Advocate for resources and removal of barriers
* Maintain ongoing dialogue with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans

Requirements

* Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition
* BLS Provider certification from the American Heart Association or American Red Cross
* Case Manager certification from the Commission for Case Manager Certification (CCMC) or American Case Management Association (ACMA) preferred
