RN Home Healthcare Professional Wanted

Join our team as a skilled and compassionate Home Health Registered Nurse As an After Hours RN, you will play a vital role in delivering quality patient care and collaborating with other healthcare professionals to achieve exceptional outcomes.
Key Responsibilities:
- Assess patients and families to determine skilled home care needs and develop individualized plans of care
- Provide direct patient care, including counseling, education, and support, as defined in the State Nurse Practice Act
- Collaborate with healthcare professionals to ensure seamless care coordination and promote evidence-based practice
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
- One to two years of recent acute care experience in an institutional setting
- One year of home health care experience preferred
- Recent triage experience in clinic, ALF, SNF, or Home Health
- Experience in customer service/direct member contact field preferred
- Graduate of an accredited school of nursing and current Minnesota Nursing License required
Salary Range:
$65-$85 per hour based on title and location
