Job Description

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Children's Healthcare of Atlanta. As a key member of our interdisciplinary healthcare team, you will provide direct patient care in a safe and effective manner, promoting quality patient outcomes.

Responsibilities
  • Practice (Caring for the Patient)
    • Cultivate and maintain effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care.
    • Communicate with all members of the multidisciplinary team to facilitate the care coordination process for specialty.
    • Communicate with home health agencies, third-party payor, and other community resources as needed to coordinate needs.
    • Facilitate and provide ongoing communication with patient/family, interdisciplinary staff and third parties (home health, third party payers, pharmacies, and other resources) to identify and resolve potential barriers.
    • Facilitate clinical practice guidelines at the patient level.
    • Perform outpatient and clinic care coordination and monitors patients' care between clinic and home.
    • Facilitate care via telehealth to assist with patient needs and concerns.
    • Refer cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel.
  • Development (Fostering Clinical Knowledge)
    • Participate in lifelong learning to maintain knowledge and competence that reflects current nursing practice.
    • Incorporate new knowledge into nursing practice to support desired outcomes.
    • Maintain professional records that provide evidence of competence and learning.
    • Self-evaluate professional practice in relation to performance, development, resiliency, and goals.
    • Take action to achieve goals identified during performance review, resulting in change in practice and role performance.
  • Engagement (Engaging and Leading Professionally)
    • Provide oversight for nursing care given by unlicensed personnel while retaining accountability for patient care quality.
    • Engage in ethical practices utilizing principles from The Code of Ethics for Nurses.
    • Maintain therapeutic and professional relationships with appropriate role boundaries.
    • Contribute to a supportive and healthy work environment.
    • Consult, contribute to, and coordinate care with interdisciplinary healthcare team members.
    • Demonstrate investment in others through teaching/coaching/helper roles.
    • Demonstrate support of shared leadership through awareness of initiatives and providing input.
  • Outcomes (Driving Excellent Results)
    • Integrate evidence and research findings into clinical judgement and nursing practice.
    • Participate in monitoring activities within work environment to maintain a culture of safety.
    • Identify and communicate hazards and errors.
    • Participate in quality and process improvement activities to improve patient outcomes and workflow.
    • Ensure equipment is in working order to provide safe patient care.
    • Demonstrate commitment and flexibility through times of change.
    Requirements
    • Complete orientation and initial assessment of competency successfully by determined due date.
    • Complete mandatory education and annual competency assessment requirements as defined by unit and system by due date.
    • Achieve 75% attendance at staff meetings if applicable.
