Care Manager

We are seeking a skilled Care Manager to join our team at Belvedere Home Care. As a key member of our staff, you will be responsible for providing case management and service coordination to individuals with disabilities and seniors.
ResponsibilitiesKey responsibilities include:
- Developing and implementing individualized service plans to meet the unique needs of our clients
- Conducting regular face-to-face meetings with clients to assess their needs and progress
- Collaborating with healthcare professionals and community agencies to ensure seamless care coordination
- Maintaining accurate and up-to-date records of client information and service plans
- Providing education and support to clients and their families to promote independence and self-advocacy
To be successful in this role, you will need:
- A degree in a relevant field, such as social work, gerontology, or a related field
- At least one year of experience in case management or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
