Key Responsibilities:
  • Create and maintain a daily activity calendar and distribute it to residents monthly
  • Develop and implement an activities program that meets federal and state regulations
  • Participate in resident care plans and other relevant meetings
  • Attend daily team meetings and collaborate with colleagues to ensure seamless service delivery
  • Document resident participation in activities and maintain accurate records
  • Plan and execute quarterly family nights and other social events
  • Deliver and read mail to residents as needed
  • Assume responsibility for scheduling and coordinating social programs for the Skilled Nursing Facility
  • Work closely with residents, family members, and community personnel to understand their needs and preferences
Requirements:
  • High school diploma or equivalent required; degree in a related field preferred
  • Minimum 2 years of experience in a similar role or related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information
  • Physical ability to perform duties and responsibilities with or without reasonable accommodation
