Job Summary

We are seeking a compassionate and skilled Certified Nursing Assistant/Home Health Aide to join our team at ChristianaCare HomeHealth. As a member of our Community Health Service team, you will work under the supervision of a registered nurse and/or therapist to provide high-quality patient care in the comfort of patients' homes.

Key Responsibilities
  • Follow a Plan of Care developed by a registered nurse and/or therapist to provide personalized patient care.
  • Adhere to infection control policies and HIPAA guidelines to ensure patient confidentiality and safety.
  • Assist with daily living activities, such as personal care, light housekeeping, and meal preparation, as outlined in the Plan of Care.
  • Accompany patients to physician appointments or clinics, providing emotional support and assistance as needed.
  • Participate in simple exercise programs, apply prosthetics and braces, and assist with ambulation and transfers, as outlined in the Plan of Care.
  • Provide stimulation and encouragement to patients, helping them achieve their goals and maintain independence.
Requirements
  • High school diploma or equivalent.
  • Completion of a CNA or Home Health Aide training program of 75 hours or greater.
  • Ability to work independently in clients' homes, with maturity and empathy towards patients' needs.
  • Effective communication skills, including reading, writing, and understanding and following directions.
