Job Summary

We are seeking a compassionate and skilled Certified Nursing Assistant/Home Health Aide to join our team at ChristianaCare Home Health. As a member of our Community Health Service team, you will work under the supervision of a registered nurse and/or therapist to provide high-quality care to patients in their homes.

Key Responsibilities
  • Follow a Plan of Care developed by a registered nurse and/or therapist to provide personalized care to patients.
  • Adhere to infection control policies and HIPAA guidelines to ensure patient confidentiality and safety.
  • Perform various tasks as outlined in the Plan of Care, including personal care, light housekeeping, meal preparation, and accompanying patients to physician appointments or clinics.
  • Assist with simple exercise programs, application of prosthetics and braces, ambulation, and transfers as needed.
  • Provide emotional support and encouragement to patients to help them achieve their goals and maintain independence.
Requirements
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant/Home Health Aide training program of 75 hours or greater.
  • Ability to work independently in clients' homes and possess a mature and empathetic attitude towards patient care.
  • Excellent communication skills, including reading, writing, and ability to understand and follow directions.
