We are seeking a highly skilled and dedicated Physical Therapist to join our team at Sky View Rehabilitation and Health Care. As a Physical Therapist, you will play a vital role in helping our residents achieve their highest level of physical, mental, and social well-being.
Job Summary:The Physical Therapist will be responsible for planning, organizing, developing, and implementing physical therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. The ideal candidate will possess excellent communication, customer service, and organization skills, with the ability to work well under pressure and in a team environment.
Responsibilities:- Assist in planning, organizing, developing, and implementing physical therapy services to ensure the highest degree of quality resident care.
- Develop, implement, and evaluate individualized therapy plans to help residents achieve their goals and maintain their highest level of functioning.
- Interpret and instruct in physical therapy policies and procedures to personnel, residents, family members, and other stakeholders as necessary.
- Participate in discharge planning, development, and implementation of resident care plans, resident assessments, and other related activities.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain accurate treatment records, resident files, and progress notes, and maintain an adequate liaison with families and residents.
- Current license and registration to practice as a Physical Therapist in the State of NY.
- Current malpractice insurance and license in good standing.
- Dedicated to providing extraordinary, compassionate care as a Physical Therapist.
- Energetic, enthusiastic, committed, and passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
- Patience, a cheerful disposition, and willingness to assist any residents and coworkers.
- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
