Physical Therapist PRN

Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility, is seeking a Physical Therapist to join our team. As a PRN Physical Therapist, you will play a vital role in helping residents achieve their highest level of functioning and independence.
Responsibilities- Assist in planning, organizing, developing, and implementing Physical Therapy services to ensure the highest degree of quality resident care.
- Develop and implement individualized therapy plans to help residents achieve their goals and maintain their highest level of physical, mental, and social well-being.
- Interpret and instruct in physical therapy policies and procedures to personnel, residents, and family members as necessary.
- Participate in discharge planning, development, and implementation of resident care plans, resident assessments, and other necessary tasks.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
- Must possess a current license and registration to practice as a Physical Therapist in the State of NY.
- Must possess current malpractice insurance and a license in good standing.
- Dedicated to providing extraordinary compassionate care as a Physical Therapist.
- Energetic, enthusiastic, committed, and passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
- Patience, a cheerful disposition, and willingness to assist any residents and co-workers.
- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
- Covid-19 vaccinations are encouraged but not mandated.
