Job Summary

We are seeking a skilled Case Manager to join our inter-disciplinary team. As a Case Manager, you will work closely with clients to develop and implement personalized treatment plans, promoting recovery and wellness in adults with severe mental illness.

Key Responsibilities
  • Provide community-based supports, including resource development and crisis intervention
  • Collaborate with clients to set and achieve goals, promoting successful outcomes
  • Develop and maintain relationships with community resources and service providers
  • Participate in inter-disciplinary team meetings to ensure comprehensive care
Requirements

To be successful in this role, you will need:

  • A minimum of an Associate's or Bachelor's degree
  • Experience working with individuals with severe mental illness, preferably in a community-based setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
