Job Summary

AltaPointe Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a medication role. As an LPN, you will be responsible for providing high-quality patient care, administering medications, and collaborating with the healthcare team to ensure optimal patient outcomes.

Key Responsibilities
  • Administer medications to patients, ensuring accurate dosing and adherence to treatment plans
  • Conduct thorough assessments of patients' physical and mental status, identifying potential medication side effects and reporting them to the RN/physician
  • Provide education to patients and families on medication use and potential side effects
  • Collaborate with the healthcare team to develop and implement individualized treatment plans
  • Document patient information accurately and thoroughly, ensuring continuity of care
Requirements
  • Current LPN licensure in Alabama
  • Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to maintain confidentiality and handle sensitive patient information
  • Flexibility to work varying shifts, including evenings and weekends
