We are seeking a highly skilled Registered Nurse to join our team at Methodist Hospital Metropolitan in San Antonio, Texas. As a Registered Nurse Clinical Transition, you will play a vital role in ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings.
Job Summary:The top priority for this role is delivering exceptional patient-centered care. You will be responsible for obtaining a nursing assessment history, ensuring proper inpatient care unit admission, and taking appropriate action when needed. You will also complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
Responsibilities:- Obtains a nursing assessment history and ensures proper inpatient care unit admission.
- Completes and facilitates the completion of required documentation.
- Teaches patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications, and possible side effects.
- Ensures patients/families/caregivers receive copies of discharge paperwork and any other documentation needed.
- Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date.
- Registered Nurse Texas State or Compact State license required.
- Registered Nurse Diploma, or Associate Degree, or Bachelors Degree.
- 1-2yrs Admission Discharge Lounge experience or Medical Surgical/Tele experiences.
