Collaborative Patient Care Coordinator

About the Job
This challenging role requires a highly skilled RN with expertise in stepdown care. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to remain calm under pressure. You will be responsible for coordinating patient care activities, communicating effectively with patients, families, and healthcare professionals, and maintaining accurate records.

Responsibilities
Coordinate patient care activities, ensuring seamless transitions between departments
Communicate effectively with patients, families, and healthcare professionals
Maintain accurate records and reports, adhering to regulatory requirements
Collaborate with interdisciplinary teams to ensure comprehensive patient care

Requirements
Degree in Nursing (Bachelor's or Master's level) from an accredited institution
Current RN licensure in the state where Della Infotech, Inc. operates
Minimum 2 years' experience in stepdown or critical care settings
Excellent communication and teamwork skills
