Home Health Physical Therapist Role

We are seeking a skilled Physical Therapist to join our team at BAYADA Home Health Care. As a Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing exceptional care to adult and geriatric patients in their homes.
Key Responsibilities- Making home visits to assess and provide care to clients
- Conducting diagnostic tests and measurements to identify client needs
- Developing and implementing individualized care plans to promote client independence and health
- Educating clients and their families on rehabilitative care and activities necessary for optimal health and safety
- A current license as a Physical Therapist in Massachusetts
