Geriatric Care Specialist

Cleveland Clinic Health System is seeking a skilled Geriatric Care Specialist to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with physicians and other healthcare professionals to deliver exceptional patient outcomes.
Responsibilities- Conduct thorough medical histories and perform complete physical examinations to establish a record of the patient's current health status and develop a working diagnosis and treatment plan.
- Initiate appropriate lab, radiology tests, or other special tests required for evaluation of illness and scrutinize lab data to identify pathophysiologic processes.
- Perform routine procedures such as injections, immunizations, suturing, and wound care, and manage simple conditions produced by infection or trauma.
- Assist in the management of more complex illness and injury, including assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life-threatening situations.
- Prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and CCF policy.
- Participate in medical team rounds and collaborate with nursing, medical, and other healthcare team members regularly to ensure quality patient care.
- Refer patients to specialists as appropriate for consultation or for specialized health resources and treatment.
- Ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
- APRN: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings.
- PA: Nationally certified licensed healthcare provider who provides primary and/or specialty medical care in a variety of healthcare settings.
- CNS: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings.
- APRN: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
- PA: Graduate of an A.R.C. approved Physician Assistant Program.
- CNS: Graduate of an accredited CNS program.
Related clinical experience preferred.
Physical RequirementsTypical physical demands include the ability to walk and stand for long periods of time, manual and finger dexterity, and eye/hand coordination to perform physical examinations.
