Job Summary:The Outreach Case Manager will be responsible for engaging with homeless persons in various settings, identifying and linking them to mainstream resources and housing. You will work closely with clients to provide support, advocacy, and referrals to ensure they receive the necessary services to achieve stability and self-sufficiency.
Key Responsibilities:- Provide community outreach services to homeless individuals and families
- Identify and engage with clients in various settings, including shelters, schools, and community centers
- Link clients to mainstream resources and housing, including temporary safe places to reside
- Provide support, advocacy, and referrals to ensure clients receive necessary services
- Work closely with clients to develop and implement individualized service plans
- MHRT/C certification (or a level A or level B with a plan to get full certification)
- 2 years' experience in social services, with a focus on working with oppressed populations
- BSW or MSW preferred, with a degree in psychology, social services, counseling, rehabilitation, or a related field
- Ability to exert moderate physical effort, with occasional standing, walking, and lifting
- Valid driver's license and a good driving record
