Lead Clinical Nurse for Home Health Care

Amedisys Inc is seeking a skilled and compassionate Registered Nurse (RN) to lead our home health care team as a Clinical Nurse Manager. As a key member of our team, you will be responsible for overseeing the quality of patient care and clinical performance of nurses and aides, ensuring that our patients receive the highest level of care in the comfort of their own homes.

Key Responsibilities:
  • Recommend employment decisions for staff to the director of operations, including hiring, firing, advancing, promoting, or changing status.
  • Oversee clinical operations for the care center, ensuring that all patients receive high-quality care.
  • Review requests for services and determine patient eligibility/suitability for home care services.
  • Work with intake to coordinate patient referrals and admissions.
  • Oversee scheduling, payor verification, and precertification requirements.
  • Review documentation of other staff members and ensure that any missing, incomplete, or untimely documentation issues are resolved.
  • Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing a Plan of Care (POC).
  • Regularly evaluate the needs of patients and make necessary revisions to the plan of care in collaboration with clinicians.
  • Participate in the on-call process and assist with training of staff to ensure quality care, compliance, and fiscal responsibility.
  • Participate in the investigation and resolution of patient/family/physician concerns.
Requirements:
  • Associate or bachelor's degree in nursing.
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience in home health or hospice.
  • One year of RN management experience.
  • Current CPR certification.
