Job Title: Full Time Registered Nurse- Home Health

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Full Time Registered Nurse- Home Health. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Collaborate with primary caregivers to ensure optimal functioning and provide education on patient care.
  • Coordinate care with other healthcare professionals, including physicians and therapists.
  • Document patient progress, including medical interventions and measurable goals and outcomes.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • Medicare Skilled Nursing experience preferred.
  • Basic understanding of Oasis.
  • 1-year RN experience in a healthcare setting.
