Job Overview:
We are seeking a bilingual professional to support case management services, with travel to multiple sites, as part of our team at TAG MedStaffing.
Key Responsibilities:
- Conduct comprehensive assessments using standardized tools such as the PHQ-9, MSSI-SA, and GAD-7.
- Refer clients to mental health professionals within a timely manner, ideally within five business days.
- Coordinate and facilitate regular group counseling and education sessions.
- Follow up on mental health and substance use treatment plans on a monthly basis.
- Document all assessments, referrals, and interactions in our database.
- Provide case management and assist with care plans to promote healthy aging.
- Prepare required documentation and cover for the Clinical Manager as needed.
- Handle more complex cases as assigned.
Qualifications:
- LMSW Degree with a minimum of two years of experience.
- Bilingual in English/Spanish or English/Chinese.
- Experience working with the geriatric population, mental illness, and substance use.
- Proficient in assessments and care coordination.
- Strong computer skills, especially in Microsoft Office Suite (Word, Outlook, Excel).
