We are seeking a highly skilled and compassionate Patient Care Manager RN to join our team at SunCrest Home Health, a part of LHC Group.
About the Role:
- Provide clinical leadership and supervision to our patient-facing teams.
- Ensure high-quality patient care and support to patients and their families.
- Develop and implement effective care plans in collaboration with the healthcare team.
- Coordinate patient referrals, clinician assignments, and ongoing insurance approvals.
- Ensure compliance with applicable laws, regulations, and agency policies.
Responsibilities:

Qualifications:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
