Schedule

We offer a standard schedule of Monday through Friday, 8am to 4pm. This allows for a healthy work-life balance and enables you to plan your personal time effectively.

Responsibilities
  • Provide education and consultation to individuals and staff regarding medical health, medication, and medical topics. Offer one-on-one training in specific areas tailored to individual needs.
  • Develop and conduct groups and trainings for individuals and staff on general areas of health and wellness, including nutrition, exercise, and smoking cessation. Assist with accessing community resources that support and address health and wellness efforts.
  • Offer oversight, guidance, and assistance to individuals in understanding their medical conditions. Meet with individuals and staff to discuss medical diagnoses, conditions, causes, symptoms, complications, and treatment options.
  • Perform direct medical care as appropriate and ordered by a physician, which may include monitoring vitals, wound care, etc. This includes providing interventions specific to an individual's treatment plan for medical/health and wellness goal areas.
  • Review all medical documentation for each assigned individual and provide medical care consultation and coordination as needed. Assist with coordinating overall medical care for individuals and ensuring follow-up appointments are scheduled as necessary. Ensure compliance with MAP.
  • Offer medical consultation and direction as an active member of assigned committees, including the Agency's risk committee and Health and Wellness committee.
  • Provide side-by-side interventions to individuals focusing on education and skills training related to health and wellness issues. Assist the team and individuals in methods for ongoing assessment of symptoms and treatment effectiveness, identifying appropriate responses/actions.
Salary

The estimated salary for this role is $74,680 per year. This compensation reflects the value we place on our professionals' expertise and dedication.
