We are seeking a highly motivated Physical Therapist to join our team at JAG Physical Therapy. As a skilled rehabilitation specialist, you will be responsible for assessing patients' strengths and weaknesses to develop effective treatment plans aimed at improving their functional independence.
You will collaborate with an interdisciplinary team, maintain patient confidentiality, and provide high-quality care while fostering a supportive environment. Ideal candidates possess excellent communication skills, clinical decision-making abilities, and are dedicated to patient success.
The successful candidate will be forward-thinking, exceling at assessing patients' strengths and weaknesses and devising creative and effective treatment plans. Therapists must practice discretion and maintain patient confidentiality. They must operate in a high-energy, positive-thinking environment and offer patients positive encouragement to achieve their goals.
As a member of our team, you will utilize your training and skills to aid patients in the goal of healing, recovering, improving pain level/tolerance, and maximizing functional independence. You will collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers. This includes initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
You will also prepare for evaluations in accordance with department policies/procedures and standards of practice. This includes obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses. You will select and administer tests and measures and synthesize evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
Additionally, you will establish treatment plans based on evaluation results, goals, and best practice. You will collaborate with interdisciplinary teams, patients, and patient support systems. You will assess and re-assess the need for modifications to the plan of care and goals. You will identify the need for consultations and/or refer patients to another healthcare professional or health care services.
Our company offers a competitive salary range of $80,000 - $120,000 per year, depending on experience.
