Physical Therapist Job Opportunity in Home Health Setting

We are seeking a skilled Physical Therapist to join our team in a home health setting in Cromwell, CT. This is a 25-week assignment with day shifts, offering a great opportunity to enhance your clinical skills and build your dream career.

Job Summary

As a Physical Therapist, you will be responsible for providing high-quality patient care in a home health setting. You will work with patients to develop and implement individualized treatment plans, focusing on improving their physical function and overall well-being.

Responsibilities
  • Assess patients' physical function and develop treatment plans to improve their mobility and independence
  • Provide one-on-one therapy sessions to patients in their homes
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Document patient progress and adjust treatment plans as needed
Requirements
  • Graduate of an accredited Physical Therapy program with an active PT license in the state of CT
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Flexibility and adaptability in a fast-paced home health setting
Preferred Qualifications
  • Experience working with geriatric patients
  • Knowledge of current Medicaid billing regulations
  • Strong organizational and problem-solving skills
