Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Sigma Healthcare. As a Physical Therapist, you will play a vital role in helping our patients achieve their goals and improve their quality of life.

Job Summary:

As a Physical Therapist, you will be responsible for evaluating patients, developing treatment plans, and working with our interdisciplinary team to provide comprehensive care. You will work with patients with intellectual disabilities, using your skills to enable them to achieve their goals.

Responsibilities:
  • Provide physical therapy services to patients according to a written physician's plan of care.
  • Evaluate and assess therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
  • Direct physical therapy treatment and instruct patients/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report to the physician patient's reaction to treatment or changes in condition.
  • Identify patient and family/caregiver needs for other home health services/hospice and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for patients.
Requirements:

To be successful in this role, you must be a graduate of an accredited Physical Therapy program with an active PT license in the state of CT. You should also possess good organizational and problem-solving skills, flexibility, and teamwork. Exceptional oral and written communication skills are also essential.
