Complex Care Social Worker - Renal Dialysis

At LifeBridge Health, we are seeking a skilled Complex Care Social Worker to join our team. As a key member of our interdisciplinary team, you will play a vital role in facilitating discharge planning for patients with complex needs.

Job Summary:

The Complex Care Social Worker will work closely with patients, families, and healthcare providers to identify and address barriers to discharge. Using a multi-step escalation protocol, you will identify patients with complex needs and develop a plan to ensure timely and cost-effective discharge.

Responsibilities:

  • Conduct thorough assessments of patients' needs and develop individualized plans to address barriers to discharge.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Develop and implement strategies to reduce length of stay and improve patient outcomes.
  • Provide education and support to patients and families to promote self-advocacy and empowerment.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Master's degree in Social Work or related field.
  • Current licensure as a LCSW-C or LCPC in the State of Maryland.
  • Minimum of 3 years of experience in social work or a related field.
