Job Summary:

This role will involve working closely with patients, families, and healthcare providers to identify and address barriers to discharge. You will use a multi-step escalation protocol to identify patients who require additional support and work with the team to develop and implement discharge plans that meet their needs.

Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop individualized discharge plans
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge
  • Develop and implement strategies to reduce length of stay and improve patient outcomes
  • Provide education and support to patients and families to promote self-advocacy and empowerment
  • Participate in quality improvement initiatives to enhance the discharge planning process
Requirements:
  • Master's degree in social work or related field
  • 3-5 years of experience in discharge planning or a related field
  • LCSW/LCSW-C licensure
