We are seeking a highly skilled and licensed Nurse Practitioner (NP) / Physician Assistant (PA) to join our team at Tandym Group in New York City.

Responsibilities:
  • Review and make recommendations on consumer assessments for long-term care services and supports.
  • Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
  • Assess cases presented and utilize clinical competencies to review and validate assessment outcomes.
  • Work collaboratively to monitor and address clinical quality and gaps in care.
  • Provide clinical and educational assistance to support project initiatives.
  • Demonstrate communication skills necessary to interpret age-specific responses.
  • Participate in performance, improvement, and quality assurance activities and related meetings.
  • Provide clinical and educational support to meet program goals.
Qualifications:
  • 2+ years of relevant work experience.
  • Master's Degree from a Nurse Practitioner or Physician Assistant Program.
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license.
  • Board Certified Nurse Practitioner (NP) and/or Physician Assistant (PA).
Desired Skills:
  • Board Certified in Internal Medicine, Adult Health, Community Health, and/or Family Practice.
  • Bilingual (English and Cantonese, Mandarin, Bengali, Haitian-Creole, Russian, or Spanish).
  • Experience working with the Adult / Geriatric populations.
  • Home Care background.
  • Experience working with individuals with Physical and/or Developmental Disabilities.
  • Knowledge of Long-Term Care services & programs.
