Job Summary
NYC Health + Hospitals is seeking a skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with physicians and other healthcare professionals to ensure the best possible outcomes for our patients.
Key Responsibilities
Provide comprehensive medical and nursing care to patients and families, including preventive and supportive health services.
Manage health problems and coordinate healthcare for various patient populations, identifying and diagnosing illnesses and physical conditions, and implementing therapeutic and corrective measures.
Develop and maintain partnerships with community-based organizations and make referrals to consulting physicians and/or other healthcare professionals and community agencies, as necessary.
Evaluate and interpret patient data to plan and implement appropriate nursing interventions, consulting with physicians and other healthcare practitioners as needed.
Formulate differential diagnoses and plans of care based on medical history, physical exams, nursing assessments, and other pertinent information, identifying health and medical risks and needs.
Develop, implement, and evaluate patient plans of care, determining effectiveness and modifying as needed to achieve medical and health goals.
Order, conduct, and interpret diagnostic and laboratory tests, identifying and prescribing appropriate pharmacological and non-pharmacological interventions and therapies.
Manage patient session schedules, ensuring availability and continuity of care, and utilizing resources effectively.
Provide education, guidance, and direction to nursing staff and other healthcare team members to ensure quality and safe medical and nursing care.
Provide education and counseling to patients and families regarding health status, treatment, and progress, medication management, disease prevention, and health promotion, and maintain relevant documentation.
Requirements
A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
Current certificate as a Nurse Practitioner issued by the NYSED.
Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Preferred Qualifications
BLS, ACLS, AHA required.
APN licensure.
Preferably 3+ years of acute care hospital experience in orthopedics and/or surgery.
Excellent communication skills, combined with the ability to collaborate across functions.
Detail- and process-oriented, with the ability to context and mode switch easily, and a fast learner.
Self-driven and excited to support new technology.
Outstanding organizational abilities.
Customer service experience, with a people person approach.
