Key Responsibilities
As a Licensed Care Professional, you will be instrumental in delivering personalized nursing care at various community living sites. Collaborating with Wilder's Healthy Aging & Caregiving Services, you will be part of a dedicated team focused on enhancing the health and autonomy of adults, ensuring they receive the support needed to thrive in their community.
Your responsibilities will include providing comprehensive nursing services tailored to the needs of older adults and those with chronic health conditions. You will oversee and guide licensed healthcare personnel, facilitate training for health care technicians, assist healthcare providers in managing patient needs, evaluate clients, and monitor their medication requirements. Regular communication with an interdisciplinary team and meticulous documentation will be vital to your role.

Qualifications
A minimum of an Associate's degree is required (Bachelor's preferred), along with 3-5 years of relevant nursing experience and at least 1 year in a supervisory or training role. Candidates must possess MN Nursing Licensure. Experience in geriatric care, home health, customized living, or long-term care settings is preferred. Candidates should be self-motivated, detail-oriented, and capable of distinguishing between independent nursing actions and those requiring physician delegation. Strong organizational, problem-solving, and communication skills are essential, along with the ability to manage multiple tasks effectively. Flexibility and the ability to work with a diverse team are crucial, as is proficiency in Microsoft Office and other relevant software.
