Clinical Pharmacist Specialist Network of Care

Children's Hospital Colorado is seeking a highly skilled Clinical Pharmacist Specialist to join our Network of Care team. As a key member of our pharmacy team, you will play a critical role in ensuring the safe and effective use of medications for our pediatric patients.
Job Summary:The Clinical Pharmacist Specialist will be responsible for providing pharmaceutical care to patients of all ages, from neonates to adults, in our ambulatory care settings. This includes developing and implementing individualized medication plans, conducting medication therapy reviews, and providing education to patients and families. You will also work closely with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
Responsibilities:- Provide pharmaceutical care to patients of all ages in our ambulatory care settings
- Develop and implement individualized medication plans
- Conduct medication therapy reviews
- Provide education to patients and families
- Collaborate with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes
- Doctor of Pharmacy degree or Bachelor of Science in Pharmacy degree
- Two years of residency experience or one year of residency plus three years of work experience
- State of Colorado Department of Regulatory Agencies (DORA) Pharmacy license
- BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted)
