Position Overview:

This role focuses on delivering nutritional services and education to patients undergoing dialysis and their authorized caregivers within a clinical environment. The aim is to enhance and maintain the nutritional health of patients suffering from end-stage renal disease, adhering to LifeBridge Health's policies and applicable regulations.

Key Responsibilities:
  • Conduct thorough assessments of nutritional status to formulate tailored nutrition interventions and recommendations.
  • Execute initial assessments within the designated timeframe and perform subsequent evaluations based on patient stability.
  • Collaborate in the development of interdisciplinary care plans, ensuring timely participation following assessments.
  • Review and document medical and dietary histories, maintaining accurate records of nutritional progress.
  • Create personalized Medical Nutrition Therapy plans, including specific meal recommendations for each patient.
  • Deliver nutrition education and counseling tailored to individual patient needs, considering their medical and social circumstances.
  • Select or design educational materials suitable for diverse patient populations.
  • Monitor and evaluate patients' nutritional status, laboratory results, and adherence to dietary guidelines.
  • Adjust nutritional recommendations and care plans as necessary based on ongoing assessments.
  • Engage in discussions regarding monthly lab results with patients and caregivers, providing clear and informative reports.
  • Act as a specialist in managing mineral and bone disorders associated with renal disease.
  • Participate in anemia management and interdisciplinary meetings to enhance patient care.
  • Document all nutrition-related progress and ensure compliance with federal and state regulations.
  • Foster effective communication with the medical team and establish strong relationships with patients and their families.

Qualifications:
  • Bachelor's degree in Nutrition, Dietetics, or a related field is required.
  • Registered Dietitian credential from the Commission on Dietetic Registration.
  • State licensure in the relevant jurisdiction is mandatory.
  • A minimum of two years of professional experience in clinical nutrition is essential.
  • Experience in a dialysis setting is preferred.
  • Strong time management and communication skills are crucial.
  • Ability to think critically and work collaboratively within a team environment.
