As a Licensed Practical Nurse (LPN) at Roosevelt Care Centers, you will provide high-quality nursing care to our residents in a long-term care facility. Under the direction of a Registered Nurse (RN), you will act as a Team Leader in a unit, ensuring the well-being and safety of our residents.
Key Responsibilities:
• Provide direct care to assigned residents, including administration of medications, treatments, and monitoring of their condition.
• Complete rounds as appropriate and document pertinent information regarding resident responses and nursing interventions in their medical records.
• Monitor specific data such as medications, lab/diagnostic results, and therapies, and assist in the completion of Minimum Data Set (MDS) and care planning process under RN direction.
• Orient residents and their families to the long-term care environment and maintain liaison with families.
• Participate in committees and conferences, and orient and assist other clinical personnel as needed.
• Initiate appropriate referrals to other departments, services, and/or outside agencies for resident care.
• Provide comprehensive patient/resident care and consult with supervisors and others regarding significant changes in resident's condition.
• Ensure that medical and emergency equipment/devices, including alarms, are in operating condition and available and functioning.
• Participate in wound care rounds and assist in the orientation and training of new personnel.
• Maintain established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards, and promote adherence to Resident Bill of Rights.
Requirements:
• Ability to read, write, speak, and understand English sufficiently to perform the job.
• Graduation from an accredited school of practical nursing and a current NJ license to practice as a Licensed Practical Nurse.
• One year of experience in clinical nursing is preferred.
