About the Role

We are seeking a highly skilled and compassionate Foster Care Coordinator to join our team at UnitedHealthcare. As a Foster Care Coordinator, you will play a critical role in ensuring the health and well-being of children and youth in foster care.

Key Responsibilities
  • Assess and develop individualized care plans for children and youth in foster care, taking into account their unique needs and circumstances.
  • Coordinate and monitor referrals for psychosocial, behavioral, and medical services to ensure timely and effective support.
  • Serve as a single point of contact for families and caregivers, providing education and support to help them navigate the healthcare system.
  • Advocate for children and families to ensure their needs and choices are represented and supported by the healthcare team.
  • Collaborate with child welfare and juvenile justice systems to ensure seamless coordination of care.
Requirements
  • Master's degree in social work, nursing, psychology, or related field.
  • 2+ years of experience working in a community health setting, preferably in a healthcare role.
  • 1+ year of experience working with multi-system children and families.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
  • Ability to travel within assigned region to visit members in foster homes and other settings.
Preferred Qualifications
  • Licensed Social Worker or clinical degree.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting and arranging community resources.
