Position Summary

Under the supervision of the Clinic Manager, the Care Manager plays a crucial role in orchestrating the care and services for CIBD patients throughout their healthcare journey. This position emphasizes the effective use of healthcare resources and involves leadership within a multidisciplinary care team to ensure optimal clinical and resource outcomes. The Care Manager operates within a collaborative framework, adhering to the standards and regulations of relevant state and federal programs.


Key Responsibilities
  • Engage with patients to assess their needs and understanding of their care plans, coordinating services with external providers such as home health agencies, pharmacies, and laboratories.
  • Collaborate with both internal and external stakeholders to guarantee seamless care coordination and fulfillment of medical orders.
  • Exhibit strong self-direction and teamwork, demonstrating excellent organizational, documentation, and communication skills, along with critical thinking and conflict resolution abilities.
  • Promote a comprehensive care team model, becoming cross-trained as necessary to provide support and maintain effective interpersonal relationships.
  • Take initiative in developing and enhancing the existing bleeding disorders and thrombophilia program through community engagement, partnerships, and professional development opportunities.
Qualifications
  • Graduation from an accredited Registered Nurse (RN) program.
  • Bachelor of Science in Nursing (BSN) is preferred.
  • Possession of an active California State Registered Nurse (RN) License.
  • A minimum of two years of nursing experience in acute care or outpatient settings.
  • Basic Life Support (BLS) certification.
  • Completion of HIPAA training.
  • Willingness to work onsite.
Preferred Skills
  • Experience in Case Management.
  • Familiarity with CCS/GHPP/Medi-Cal regulations.
  • Understanding of third-party payer systems and regulations.
  • Background in hematology/oncology nursing, either in adult or pediatric care.
  • Bilingual proficiency in English and Spanish is advantageous.
