LPN Long-Term Care Specialist

The ideal candidate will provide high-quality patient care to residents in a comfortable and therapeutic environment, assisting individuals in their recovery and daily living activities.
About the Role:This position requires a skilled LPN with experience in long-term care, rehabilitation, or similar healthcare settings. The successful candidate will work closely with nursing staff, therapists, and other healthcare professionals to provide comprehensive care and develop a supportive environment for residents.
Key Responsibilities:- Deliver direct patient care under the supervision of Registered Nurses (RNs) and physicians, including administering medications, monitoring vital signs, and assisting with daily living activities such as bathing, dressing, and feeding.
- Collaborate with rehabilitation therapists to implement and support individualized care plans, encouraging residents to reach their maximum level of function.
- Maintain accurate and up-to-date patient records, including assessments, interventions, and progress notes, ensuring compliance with facility policies and regulatory standards.
- Work closely with nursing staff, therapists, and other healthcare professionals to provide comprehensive care and develop a supportive environment for residents.

Requirements:To be successful in this role, you will need:
- A current and valid LPN license in the state of practice.
- Previous experience in long-term care, rehabilitation, or similar healthcare settings.
- Graduation from an accredited practical nursing program.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Proficient in electronic medical records (EMR) and basic computer skills.
The estimated salary for this position is $60,000 - $80,000 per year, depending on experience. We offer a competitive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
