Hospice Care Nurse

About the Role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Hospice team at BrightSpring Health Services. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in their time of need.
Key Responsibilities
- Provide direct patient care to hospice patients, including pain and symptom management, and implement nursing practices following comprehensive assessments and care plans.
- Assess and provide patient and family/caregiver education, and participate in the coordination of hospice services.
- Ensure accurate and timely documentation of patient services, and evaluate patient response to treatments and medications.
- Participate in professional growth and development opportunities, quality assessment activities, and ensure compliance with state and federal legal and regulatory requirements.
Requirements
- Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
- 1 year of nursing experience required, with 3+ years preferred.
- Experience in community health, hospice, or med/surg preferred.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
