Key Responsibilities
  • Provide comprehensive nursing care to residents in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans to meet the unique needs of each resident.

  • Conduct regular home visits to assess resident needs and provide ongoing support and education.
  • Document resident care and progress in a timely and accurate manner.
Requirements
  • Current RN licensure in the state of practice.
  • Minimum 3 months of experience in Long Term Care, with 6 months preferred.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills.
