Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Interim HealthCare of the Twin Cities. As a LPN, you will provide high-quality patient care in the comfort of their own homes.

Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provide services ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN supervisor.
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Complete timely and accurate clinical notes including addressing patient's progress.

Requirements:

  • Current license as a Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
  • 2 years' experience as an LPN.
  • Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
  • CPR Certification

  • Hourly Range: $29.51 - $35.88 per hour plus weekend differential while working on Saturday and Sunday's.
