About the Role

We are seeking a dedicated and compassionate Home Health Care Specialist to join our team at Snowline Hospice of El Dorado. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.

Responsibilities
  • Coordinate and provide care to our hospice patients, ensuring their physical, emotional, and spiritual needs are met.
  • Calm and soothe patients and families experiencing strong emotions, providing empathetic support and guidance.
  • Collaborate with a team of healthcare professionals to deliver exceptional care, addressing personal care needs and promoting patients' comfort.
  • Document care and participate in interdisciplinary meetings, training, and development activities.
  • Assist with observing patients and providing support as needed.
Requirements
  • Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification, issued by the State Department of Public Health.
  • Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification.
  • One year of aide experience, with one year of hospice experience preferred.
  • Possession of a valid California Driver's License, access to a reliable automobile, and minimum car insurance coverage.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Demonstrated ability to efficiently use a computer, with prior electronic medical record (EMR) experience preferred.
  • Open to flexible scheduling, including evenings and weekends, and reliable transportation to travel throughout the service area.
