The Rehabilitation Care Physician is responsible for delivering comprehensive medical services to patients requiring follow-up care after hospitalization. This role focuses on managing adult patients in Inpatient Rehabilitation Facilities (IRFs) and skilled nursing facilities (SNFs). The primary objective is to facilitate a smooth transition for patients from hospital settings to post-acute care environments, concentrating on chronic conditions, disabilities, and other health challenges that necessitated their initial hospitalization.
Key ResponsibilitiesThe Rehabilitation Care Physician will engage in various essential tasks, including:
- Evaluating Care Plans: Conduct daily reviews of patient care plans to ensure optimal treatment and support.
- Patient Rounding: Monitor patient progress, address any concerns raised by staff, and modify treatment strategies as required.
- Admission Coordination: Collaborate with healthcare personnel to manage and respond to admission requests for skilled nursing facilities.
- Clinical Care Provision: Offer ongoing medical attention to patients needing close monitoring following hospital stays or emergency visits.
In addition to these responsibilities, the Rehabilitation Care Physician is expected to uphold a high standard of professionalism by promptly addressing inquiries and concerns from patients, families, and healthcare staff. Clear and concise documentation is also essential to ensure effective communication and care continuity.
