Purpose and Scope

Fresenius Medical Care North America is seeking a highly skilled and experienced Social Work Services Manager to join our team. As a key member of our organization, you will play a critical role in supporting our mission to provide exceptional patient care and improve health outcomes.

Key Responsibilities
  • Provide leadership and guidance to facility master social workers in an assigned area, ensuring they have the necessary tools and resources to deliver high-quality patient care.
  • Identify clinical quality improvement opportunities and develop strategies to address them, ensuring compliance with CMS Conditions for Coverage and FMS quality program requirements.
  • Manage the tactical execution of Quality and Social Work initiatives, interventions, and standardized supportive counseling and education materials with facility MSWs within the assigned area.
  • Provide general direction, social work guidance, and feedback to facility MSWs, ensuring they meet professional standards and FMCNA guidelines.
  • Develop and implement leadership, coaching, and development plans for direct reports, promoting an engaged and productive workforce.
  • Manage the staffing of Social Work Services to ensure master social workers coverage, including enlisting coverage during times of leave, vacations, census increases, and vacancies.
  • Provide orientation and training to new master social workers, ensuring they meet policies and procedures and have the necessary skills to deliver high-quality patient care.
  • Collaborate with management staff to achieve effective inter-disciplinary, intra-disciplinary, and clinic relationships.
  • Monitor and audit Social Work Progress Notes, Comprehensive Interdisciplinary Assessments, Plans of Care, and other documentation to ensure compliance with CMS Conditions for Coverage and FMS policies.
  • Ensure all master social workers within the assigned area have current licensure and maintain continuing education hours as specified by the Association Social Work Board and state licensure regulations.
Requirements
  • Masters in Social Work required
  • State Specific Licensure required
  • 2-3 years dialysis experience required
  • 3+ years' supervisory or project/program management experience preferred
  • Licensure to provide therapeutic counseling and supervision, preferred
  • Proficient with Microsoft applications, including Outlook, Word, Excel, PowerPoint, and other web-based applications

Day-to-day work includes desk and personal computer work and interaction with patients, facility staff, and physicians. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials. The position requires travel between assigned facilities and various locations within the community. Travel to Regional, Division, and Corporate meetings may be required.
