Key Responsibilities
  • Lead and Manage Health Navigators: Serve as the manager for the Health Navigators in your market, providing full managerial responsibilities and ensuring they have the resources and support they need to succeed.
  • Develop Strategic Relationships: Develop and maintain strong relationships with partner clinic administrators, providers, and staff to address questions or concerns and ensure seamless communication.
  • Recruit and Train Team Members: Partner with Market and Regional Leaders to hire, train, and manage a team of Health Navigators, providing them with the skills and knowledge they need to excel in their roles.
  • Design and Execute Training Programs: Design and execute healthcare education and professional development trainings for the Health Navigators in your market, ensuring they have the knowledge and skills they need to provide high-quality care.
  • Establish Communication Channels: Create clear and efficient communication pathways between Health Navigators and the central office, ensuring that all stakeholders are informed and up-to-date.
  • Monitor and Analyze Performance Data: Track and analyze operational and clinical data and key performance indicators, identifying areas for improvement and implementing changes as needed.
  • Troubleshoot Operational Challenges: Troubleshoot operational and clinical challenges and offer support to the Health Navigators, ensuring that they have the resources and guidance they need to overcome obstacles.
  • Contribute to Process Improvement: Contribute to ongoing process improvement efforts to enhance our operational outcomes and ensure that we are providing the best possible care to our patients.
  • Visit Primary Care Clinic Locations: Make regular visits to our primary care partner clinic locations to visit Health Navigators and check-in with key stakeholders at the clinic, ensuring that we are building strong relationships and providing excellent care.
Requirements for This Role
  • Passion for Community Development: You must love your community and want to see it thrive, with a deep understanding of the challenges and opportunities facing rural America.
  • Excellent Communication Skills: You must be an excellent communicator, able to develop strong relationships with stakeholders and communicate effectively with Health Navigators and other team members.
  • Strong Leadership Skills: You must be an excellent manager, able to create a following and inspire others to achieve their best.
  • Clinical Experience: You must have clinical experience working with patients and medical professionals, with a strong understanding of healthcare operations and management.
  • Problem-Solving Skills: You must have superior problem-solving skills, able to troubleshoot operational and clinical challenges and offer support to Health Navigators.
  • Adaptability and Flexibility: You must be open to a fast-paced startup environment, where change is the norm and adaptability and flexibility are essential.
  • Self-Starter: You must be a self-starter, comfortable with a remote working environment and able to work independently with minimal supervision.
  • Education and Certification: You must hold a bachelor's degree and have a clinical license (Nursing or Social Work) is preferred.
  • Full-Time Role: This is a full-time role, requiring a commitment to working a standard full-time schedule.
