Job Summary

We are seeking a highly skilled and compassionate Physician to join our team at Hospice of Holland. As a key member of our medical staff, you will provide expert medical care and education to patients and families in a variety of settings, including inpatient, outpatient, home, and facility-based care.

Responsibilities
  • Provide medical consultation to Hospice personnel and assume primary physician responsibility for patients without a qualified primary care physician.
  • Participate in admission and re-certification processes and weekly interdisciplinary team meetings.
  • Make visits to patients as needed, addressing acuity, date of admission, pain and symptom management, and regulatory requirements.
  • Consult with attending physicians regarding symptom control and clinical care plan issues, and serve as a consultant to community physicians on potential Hospice patients.
  • Collaborate on standing protocols, practice standards, and outcome measurements for the Palliative Care program.
  • Review patient medical diagnosis, prognosis, medications, procedures, and clinical course to ensure cost-effectiveness and Medicare compliance.
  • Develop and maintain effective relationships with healthcare professionals in the community, serving as a source of end-of-life medical expertise and promoting the benefits of hospice care.
  • Participate in establishing a call schedule for 24/7 coverage of physician services.
  • Ensure compliance with state, federal, and accreditation regulations and standards.
Requirements
  • Current license to practice medicine in Michigan (M.D. or D.O.).
  • Board eligibility/certification in internal medicine, family medicine, oncology, emergency medicine, or hospice and palliative medicine.
  • Board certification in palliative medicine within 2 years of hire, with fellowship training preferred.
  • At least 3 years of medical practice experience.
