Home Health Registered Nurse Opportunity

LHC Group, a leading provider of post-acute care, is seeking a skilled Registered Nurse (RN) to join our Home Health team in Atlanta, Georgia.

About the Role

As a Registered Nurse in Home Health, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements
  • Current RN licensure in the state of Georgia
  • Minimum 1 year of experience in home health or a related field
